1. What if there is a problem with my equipment?
If your equipment is still under warranty, bring it in its original packaging to the GACHI office nearest you. We will contact the equipment vendor and arrange for it to be repaired or replaced free of charge.

2. How long will it take to repair my equipment?
Most equipment can be repaired within two weeks.

3. What is the minimum age a person can apply?
You must be at least 5 years old to receive equipment from GATEDP.

4. Is the equipment mine to keep?
No, the equipment is on loan to you from the state of Georgia. You may keep it as long as you continue to reside in Georgia.

5. What if my equipment is lost or stolen?
Notify GACHI at 404-297-9461 (voice/TTY) or 1-888-297-9461 (voice/TTY) immediately if your equipment is lost or stolen. If your equipment is stolen, you must obtain a police report and contact your insurance agency, designating GACHI as the beneficiary.

6. I don't think my equipment is working properly. What should I do?
Contact GACHI at 404-297-9461 (voice/TTY) or 1-888-297-9461 (voice/TTY), and one of our staff members will examine your equipment to determine if it should be sent back to the vendor for repairs.

7. My daughter received a TTY (text telephone) from GATEDP several years ago. Now, she’s about to leave from college. Can she take the equipment with her?
Only if she will be attending school within the state of Georgia. If not, she must return it within 30 days.

8. How can I get a GATEDP application?
• Call 404-297-9461 (voice/TTY) or 1-888-297-9461 (voice/TTY).
• Visit one of GACHI’s five offices (Decatur, Columbus, Rome, Augusta and Macon)
• Download an application from this Web site (see below).
• Attend an informational GATEDP workshop. Note: GATEDP workshops are held monthly at various locations throughout the state. For a workshop schedule, call 404-297-9461 (voice/TTY) or 1-888-297-9461 (voice/TTY).


10. What documents do I need to send with my application?
• Proof of income (copy of your most recent pay stub, government benefit check stub/letter OR W-2 form)
• Certificate of Georgia residency (copy of your Georgia driver’s license OR ID card) • Proof of phone service in your home (copy of your most recent telephone bill) • Certificate of need form (to be filled out by your physician)


11. When can I apply for new equipment?
You may reapply for new equipment every five years, or sooner if your disability changes.
