Application Process
& Forms
Ways to obtain a GATEDP application:
• Call 404-297-9461 (voice/TTY) or 1-888-297-9461 (voice/TTY)
• Visit one of GACHI’s five offices (Decatur, Columbus, Rome,
Augusta and Macon)
• Download an application from this Web site (see below)
• Attend an informational GATEDP workshop Note: GATEDP workshops
are held monthly at various locations throughout the state. For
a workshop schedule, call 404-297-9461 (voice/TTY) or 1-888-297-9461
(voice/TTY).
Required documents
The following documents must be sent with your application (please
note that missing or incomplete information will delay your application
review):
• Proof of income (copy of your most recent pay stub, government
benefit check stub/letter OR W-2 form)
• Certificate of Georgia residency (copy of your Georgia driver’s
license OR ID card)
• Proof of phone service in your home (copy of your most recent
telephone bill)
• Certificate of need form completed by your physician (see below)

How GATEDP processes
your application:
1. A GATEDP staff member (called an “intake specialist) date stamps
and pre-screens your application.
2. The GATEDP coordinator evaluates your supporting documents
and makes an eligibility decision.
3. If you are found eligible, the coordinator will assign you
a GATEDP trainer. If you are found ineligible, you will receive
a letter from GATEDP explaining the reason(s) for your rejection.
4. Your assigned trainer will contact you to schedule an interview.
During this interview, your trainer will help you select the best
equipment for yor needs and teach you how to use it properly.
5. Approximately 90 days after you receive your equipment, a GATEDP
assistant will conduct a follow-up evaluation with you.
